Sir,

A 40-year-old male hypertensive patient underwent magnetic resonance imaging brain for a history of fall while driving. Axial flair images showed small ischemic foci in bilateral frontoparietal deep white matter and age-related cerebral atrophy [Figure 1]. Angiogram neck showed significant narrowing of the left distal middle cerebral artery (MCA) with paucity of flow in distal branches. An abnormal vessel was seen arising from the temporal branch of the left external carotid artery at C1 level just after bifurcation of the common carotid artery supplying the left MCA territory after coursing along the left cerebral convexity [Figure 2]. Probably, this anomalous artery not described in literature previously prevented a stroke in our patient.{Figure 1}{Figure 2}
